Complimentary Transformation Call

The purpose of the call is to learn about your unique self and health issues and discover your readiness to make long-lasting changes. This is also a chance to get to know each other and see how we resonate since we will be working together for the next few months. First, you will fill out a form, and once I review it, you’ll receive an email to schedule the call.

This is a 90-minute in-depth review of your health, issues, and goals for the next steps.

Before the visit, you will be sent the necessary forms to collect the entire health history via a patient portal. The forms must be completed 5 days before the start of the appointment. This gives me enough time to review your history and make the best of our time together.

We will review your health goals, history, life history, imbalances, next steps, and recommendations that include the necessary lab tests needed.

After the initial consultation, you can join one of the 2 coaching programs that best fit your goals and needs.

If you decide to move forward and join the program within 2 days, which I hope you do because I know it will bring harmony, balance, and health into your life. A $299 credit will be applied towards either package.

Cancellation Policy

If you are sick or have an emergency and need to cancel, please reach out to me via email or telephone to inform me. Since time is of the essence, I kindly ask that you cancel a new appointment 72 hours in advance and a coaching appointment 24-hour in advance. If you cancel in less than the presented time frame, there will be a $50 fee for initial appointments and a $25 cancellation fee for coaching visits.

Why do you only offer Programs?

A program provides me with more free time to show up for my clients and fully attend to their needs.

Plus, the typical 7 minute time available to medical professionals in medical facilities doesn’t get the clients heard. Neither is that time frame enough to get them the results that they so deserve.

Several studies have shown that receiving regular support, getting questions answered sooner, and getting consistent feedback results in an easier achievement to optimal health.

I love to spend distraction-free, fatigue-free time with my clients to provide them with personalized attention and the utmost transformation in every single session that they genuinely need.

Do you accept Insurance, Medicaid, or Medicare?

I do not accept insurance, Medicaid, or Medicare. This helps me provide you with the best care possible without restrictions. 

You do have an option to use your Health Savings Account (HSA) to pay for your consultations and/or lab fees.

What specialty labs do you order?

There is a list of specialty labs we can order from. Note** For some labs, I can provide my recommendation that you can ask you medical provider to order specific labs from Quest Diagnostics and LabCorp.

Additionally, I also order specialized tests for gut health, hormonal health, mold, food sensitivity testing, heavy metals, and genetic testings through GI Map, DUTCH, CYREX, Genova, Great Plains, MicrobiomeLabs, etc.

How much do labs cost?

The conventional labs from Quest Diagnostics and LabCorp can be billed by insurance since they can be ordered by your physician.

Specialized Functional Lab Testings are primarily cash-based. Costs are transparent, and they can range anywhere from $150-$2000. This is an additional investment on your part, and the cost depends on your recommendations and needs.

Can I share prior labs with you?

Absolutely, any recent labs you have can be shared with me. You will have the chance to upload them directly to the client portal once you start with me. Please do not attempt to mail anything. As everything is an online service.

What about vitamins and supplements?

Your recommendation for vitamins & supplements will vary, and this is a very personalized approach.

After our consultation and review of the tests, I will provide my exact recommendation of what will benefit you and what is unnecessary.

Supplements can also vary from time to time, based on the stage of healing you are in.

Rest assured, you will receive a 15% discount on my professional medical-grade dispensary.

The discount and dispensary will provide a considerable discount off of the MSRP price but be sure to budget at least $150-$300 for supplements for the first few months.

How does telemedicine work?

Telemedicine is the modern and convenient solution to receive quality care.

You do not have to spend hours waiting at the medical offices to see someone anymore.

My practice was built for virtual visits around the globe to easily see anyone who would like to work with me.

As long as you have a working computer we can see each other virtually. You do not need to install any software for your visits.

You’ll receive all the information via email shortly after your appointments are booked.

I recommend preparing ahead and having a quiet place to sit where the audio is good and you have good internet service, so we don’t lose connection.

What are your fees?

I have a couple of different programs depending on your needs.

One time initial consultation – $309

3-month personalized nutrition, exercise, and lifestyle program for $1999 or a payment plan of $799 x 3

And finally…

6-month transformation program for $4999 or 6 payments of $875

What is your cancellation policy?

Please be respectful and courteous, and be sure to cancel or change your new appointments at least 72 hours in advance and your follow-up appointments 24 hours in advance.

I understand that life happens – emergencies or last-minute changes or you fell sick, and in that case, be sure to send me an email or call at 910-338-9819 to inform me of any changes, as soon as possible.

There is a fee associated with no show, late-cancellation, or rescheduled appointments.

It includes a $50 fee for the initial new appointment and $25 for follow-up appointments canceled or rescheduled less than the time frame given above.

You will be able to reschedule your appointments in the patient portal 72 hours in advance.

How do I schedule and appointment?

If you are a new client, then I ask that you fill out a form here first. After our call, you will receive a link to schedule your appointment.

If you are in the program, you will choose and schedule your appointments from the client portal.

If you purchase the initial consultation, you will receive an email with forms to fill out, and after everything I submitted, you will receive a link to schedule your first appointment.

What states can you see clients in as an RD?

Currently, as an RD, I can see a client for medical nutrition therapy in the following states: Alaska, Arizona, California, Colorado, Connecticut, Florida, Hawaii, Idaho, Indiana, Massachusetts, Michigan, New Hampshire, New York, North Carolina, Oklahoma, Oregon, Pennsylvania, Texas, Utah, Vermont, Virginia, Washington, West Virginia, Wisconsin, and Wyoming.

If you are in any other states then be sure to reach out as we can discuss the process. 

What countries can you accept clients for your coaching services?

As a coach, I can see you anywhere in the world. So, feel free to reach out. Fill out the form, and we can discuss the next steps.

My programs are designed as coaching programs. However, depending on your need, I highly recommend that you fill out the form first and do a consult or reach out directly if you have just a few questions at info@simplysupercharged.com.

Do you prescribe medications?

No. I do not. I do not have the authority to prescribe medications.

However, suppose there are any specific recommendations or concerns. In that case, I am happy to discuss those with your medical provider and come to a solution.

Is my information safe?

Absolutely!

I use HIPPA compliant software to store all your health information. Your information is not shared with anyone without your permission.
